Nomination Details ISP Soccer

To nominate for the Inclusive Sports Indoor Soccer Carnival or the Outdoor integrated soccer carnival, please complete the following nomination form.

If you do not have all the required information (i.e. mobile phone numbers) please enter TBA and then email the information to the ACC as soon as you are able.

Once your nomination form is submitted you will receive a confirmation email from the ACC. If you do not receive a confirmation email then your form was not submitted correctly and you should contact the ACC office.

NOTE: Number of teams per school may be limited for Indoor Soccer (Lords Recreation Centre), due to limited court availability.
