Review Day + SMC Lunch - Registration Form

Complete the form below to register for the following:

1. Awards Breakfast Function
2. SMC End of Year lunch
3. Car Bay Reservation

Only one form per school should be submitted by the Sports Coordinator. Principal PA's should consult with their Sports Coordinator regarding school staff attendance and registration.

AWARDS BREAKFAST FUNCTION:  Cost - $49.70 pp. Includes two course breakfast and pastry selection with brewed coffee, tea and orange juice.

SMC END OF YEAR LUNCH: Cost - $65.00 pp. Includes morning tea (prior to SMC meeting), gourmet lunch, beer, wine & soft drinks.

Schools will be invoiced by the ACC for the cost of their registration. Attendance is open to all sports coordinators and other school staff involved in ACC sport.

RESERVED CAR BAY: Cost - $20.00 per bay.  Schools can book one reserved car bay per school and will be notified should more bays become available.
