The ACC will be using images of participating players for social media and general publications. 

School sport staff are required to complete the following form by Friday 5 June for Year 7 carnivals, and Monday 27 July for Year 10 carnivals, if they have a student that does not give consent to be photographed.  

If there is more than one student, please complete a second form.
