Inclusive Beach Carnival nomination form

To nominate for the Inclusive Beach Carnival please : 

* complete all fields as required per below, and SUBMIT

* if you do not have all the required information, please enter TBA and email the information to the ACC as soon as you are able.

* once the below information is submitted, you will receive a confirmation email from the ACC. If you do not receive an email from the ACC, then your form has not been submitted correctly, and please therefore contact Cherie Pirnie, at the ACC office.

NOTE

* the event will see students from various schools mixed to form social groups for the activity element of the day

* please ensure you upload the nomination form, as provided by the ACC
