Player Nomination Form - Boys

Player nominations will open and close in Term 2. 

Player nominations for 2026 are now open.

Please see your school's Head of Sport or Sport Coordinator to seek their endorsement of your nomination. Nominations are open to players from Year 9 to Year 12.

The cost of nomination is $27.50 and this is payable to your school Head of Sport or Sport Coordinator.

Once you have the approval of your school's Head of Sport or Sport Coordinator, please complete the player nomination form below.  

Note the information regarding team selection trials. Trials are held in Term 3. Trial dates and locations will be published once confirmed. 

Once your nomination is submitted you will see a confirmation message and should receive a confirmation email. If you do not receive confirmation, contact rebecca.abe@cewa.edu.au at the ACC office to check whether your form was received.

Please add via file attachment any additional information that you may have to support your nomination. This would include expanding on the highest level of basketball you have played and any notable achievements. Students from regional areas that are not able to make the selection trials in Perth can submit with their nomination a reference/endorsement from the Basketball WA regional officer in their town/area. The selectors can use this as a tool for selection but this is only an option for regional players that cannot get to trials in Perth. Allowed file attachments are Word, Excel, PDF or PPT.
